Secondary Transition and Graduation

Transition services are a coordinated set of activities that focus on activates or courses of study that will facilitate students’ movement toward their post-secondary goals.

Begin with the End in Mind

ESC 6 can offer technical assistance and training regarding transition services and graduation in order to assist educators in creating opportunities for successful futures and post school outcomes for youth with disabilities. The assistance can be provided through forums, on-site training, regional trainings, e-mails, and telephone conferences.
